Transcript Request

To request your transcript for college, careers, or personal purposes, please mail a request containing the following information.

• Your full legal name (and your maiden name, if applicable)
• Your birth date
• The year you graduated
• Where to send your transcript, including the person or office to address it to and their complete address

Your request should include your signature.
